Director of Nursing (DON)
Personal Care & Memory Care Community
Location: Pennsylvania
Reports To: Administrator
Position Summary
The Director of Nursing (DON) is responsible for the overall clinical leadership, regulatory compliance, and delivery of high-quality resident care in a Personal Care and Memory Care setting. This role ensures services are provided in accordance with Pennsylvania Department of Human Services (DHS) regulations, promotes resident safety and dignity, and supports person-centered care for older adults, including those with cognitive impairment.
Essential Duties & Responsibilities
Clinical Leadership & Resident Care
- Oversee all nursing services and ensure residents receive care consistent with assessed needs, service plans, and physician orders
- Provide clinical oversight for medication management, treatments, wellness monitoring, and infection control
- Ensure memory care programming supports residents with dementia through evidence-based practices
- Participate in resident assessments, care planning, change-of-condition evaluations, and family conferences
- Manage budgetary and resource allocation for the nursing department
Regulatory Compliance & Quality Assurance
- Ensure compliance with Pennsylvania DHS regulations for Personal Care Homes (55 Pa. Code Chapter 2600)
- Maintain survey readiness and oversee corrective action plans as needed
- Monitor quality indicators and implement performance improvement initiatives
- Maintain accurate clinical documentation and resident records
Staff Leadership & Development
- Recruit, train, supervise, and evaluate nursing staff including LPNs, Med Techs, and Nurse Aides
- Ensure appropriate staffing levels and competency-based training programs
- Provide coaching, education, and performance management to support clinical excellence
- Promote a culture of accountability, teamwork, and compassionate care
Medication & Clinical Systems Management
- Oversee medication administration systems, audits, and pharmacy coordination
- Ensure proper storage, documentation, and disposal of medications
- Implement infection prevention protocols and oversee outbreak response
Communication & Collaboration
- Serve as clinical liaison with physicians, hospitals, families, hospice, home health agencies, and regulatory bodies
- Participate in interdisciplinary team meetings and care conferences
- Support resident and family education regarding health conditions and care plans
Risk Management & Safety
- Lead incident investigations, falls prevention strategies, wound management, and emergency preparedness planning
- Ensure staff competency in first aid, CPR, infection control, and dementia care practices
- Ensure QAPI plan is in place
- Promote resident rights, dignity, and person-centered approaches
Qualifications
Education & Licensure
- Current, active Licensed Practical Nurse (LPN) minimum required; Current, active Registered Nurse (RN) license in the Commonwealth of Pennsylvania preferred
- Associate’s degree in nursing or bachelor’s degree preferred
- CPR certification required
Experience
- Minimum of 3–5 years nursing leadership experience in senior living, long-term care, assisted living, personal care, or memory care
- Experience working with residents with dementia and cognitive impairments preferred
- Knowledge of Pennsylvania DHS regulations strongly preferred

- Strong leadership, clinical judgment, and organizational skills
- Excellent communication and interpersonal abilities
- Ability to manage multiple priorities and regulatory requirements
- Proficiency in electronic health records and medication management systems
- Demonstrated commitment to resident-centered care
Physical & Work Requirements
- Ability to walk, stand, lift, bend, and assist residents as needed
- Availability for on-call rotation and emergency response
- Ability to work flexible hours including evenings, weekends, and holidays as required
